right - there is no DMIS2010 SP10. So you need to apply the note for DMIS 2010 SP9.
For DMIS 2011 SP you do not need the note, the table is available.
I think there is one issue in the setup:
DB_SCHEMA: SLT_TEST -> (Schema used in the SLT configuration)
You have to use here the source db schema, not the schema/configuration name, used within SLT.
