I can't comment on your second problem, but regarding the CSV file, you must make sure that the table where you want to import the data has the same number of columns as the CSV file, and you must also ensure that the data types match. It seems that perhaps the first column of your table is for the date, but the CSV file doesn't have the date as the first entry. I think the '2,2,' values before the date might be the issue (depending on how your table is set up).
